Additional contract terms
* Additional fees will apply if resident prefers to schedule additional sessions.
** Additional fees will apply if admission or other add-on fees are required by venue.
*** The Salon is available, at no charge, to the service provider for the resident. Fees associated with service are to be paid directly to the provider by resident.
**** Level of care services are determined through a process involving medical staff, the resident and family members. The exercise is conducted periodically and updated to reflect the most recent conditions to be addressed through changes in care models.
